  1. Download documents 1, 2 and 3 from the Required Documents list below and have electronic versions of the following documents, if applicable: TaxID, W-9, Sun Biz Registration, Business Licenses and Certificate of Insurance.
  2. Complete and sign documents 1 and 2 and scan them as PDF files.
  3. Complete 'Document 3' Budget Proposal (see Required Documents list below) and attached it to the appropriate Budget section in step 4.
